Apply for USAF EFMP Respite Care

To apply for Air Force EFMP Respite Care:

Air Force families who are interested in respite care and enrolled in the Exceptional Family Member Program (EFMP) call Child Care Aware® toll-free at 1-800-424-2246

Once an Airman’s eligibility is confirmed, Child Care Aware® will connect the Airman's family to the local participating agency. The agency will work individually with each family to complete the necessary application and assist with connecting the family to a local respite care provider who will meet their needs.  

Child Care Aware® will send the family the following verification forms to complete and return:

• Respite Care Priority Verification form  Respite Care Priority Verification form ( 53.15 KB )
• Deployment Verification form  Deployment Verification form ( 51.54 KB )
• Wounded Warrior Verification form   Wounded Warrior Verification form ( 79.82 KB )

Parent Documentation Needed

• Self Certification form for children listed on the application  Self Certification ( 14.76 KB )

Provider Documentation Needed

• W-9  W-9 ( 59.46 KB )
• Voided Check
• License, (if applicable)
