Requirements

Providers who want to participate in ANY of the Marine Corps Fee Assistance programs must:

• Be state-licensed
• Have had a state licensing inspection in the last 12 months

 Required Documents - Centers and Family Child Care Homes

• Completed fee assistance application (available   online or download via Microsoft Word ( 57.97 KB )  or PDF ( 219.92 KB )
• Copy of state license
• Copy of state licensing inspection within last 12 months
• Copy of valid national accreditation certificate, CDA credential, or degree, as appropriate
• W-9 Form ( W-9 ( 70.66 KB ))
• Copy of Employer Identification Number Certificate EIN Instructions ( 625.78 KB )
