After Approval Received:

Give military families your Provider ID number so that they may reference your facility on their application. A family must be approved to receive fee assistance at your facility before payments can be rendered. 

Supporting Documentation Needed

  • Child Care License
  • W-9 Form
  • Employer Identification Number (EIN) Certificate[Issued by the Internal Revenue Service (IRS)]
  • Child Care Rates for all age groups for the entire year (school year and summer). All potential discounts, registration fees, and additional subsidies accepted should be included.
  • Annual Licensing Inspection Report

Family & Provider Approval

In order for a family to be approved for fee assistance with your facility, both the family and provider must be eligible for the same programs.  If you have questions about your or your family’s program eligibility, please contact Child Care Aware® at 1-800-424-2246.
