Self Assessment Scoring

The Self Assessment Scoring Sheet is designed to be used by the staff and board of an agency interested in NACCRRA Quality Assurance. Together, the staff and board are encouraged to review the Best Practices Criteria and score each indicator as "meet", "partially meet" or "do not meet". This helps the agency assess which indicators they may need to develop, expand, or enhance services to meet the criteria, and/or write documentation or evidence to demonstrate that the indicator is being met. Some of this can be done during the 24-month submission process. In addition, the scoring sheet will enable the agency to determine for which indicators they need training or technical assistance in order to meet the criteria. This information will be used by NACCRRA to develop group training and technical assistance.
